Before starting services we need to know more about yours or your loved one’s daily routines. We conduct an in-depth home health care assessment, which takes about 45 minutes.  We will learn about you or your loved one’s lifestyle, medical conditions, personality and the kind of assistance needed that will help you or them remain in their home and independent. We will meet with you and your family jointly to determine the scope of in home care needed.  Our complimentary assessment will enable us to write a Residential Care Plan.  The In Home Care Plan is a road map for our caregivers to follow during their time with you.  The Home Care Services Plan is a customized comprehensive plan to meet the senior’s and the family’s physical, psychological and social needs. We fully explain the senior care services available, answer any additional questions, and help with the important elderly care decisions faced by the family with a loved one in need.  Then the best Caregiver is matched to the senior’s needs and follow-up supervision ensures that the plan is well-implemented and executed.
